| Hi everyone. I do not think our company has ever completed an EE0-1 form and now the subject has come up. We have over 100 employees. If we do indeed need to complete this, we will need to do a voluntary Employee Identification survey of some kind as we have no previous data. Does anyone know of where I can find a sample of what companies send out when they do this? any help would be great!
