Best Practice - Rehire or Not Expand / Collapse

Does anyone know what the best practice is whether to have an area on the termination report that asks if the exiting employee is eligible for rehire or not? What is the practice at your company? Lasy question, if you gather this type of information do you give it out if asked?

Any information provided will be greatlty appreciated. Thanks!!
