An employee just passed away and his beneficiary was his mother whom is deceased as well. What legal forms can we accept to release last payroll check and life insurance (all under $100k)? Do we just need an affidavit notarized or will we need a court order stamped and signed by a judge? Any help would be greatly appreciated.
