| I have read so many things and am very confused about what we are required to give to the employee and what we are not. My COBRA administrator says we don't need to provide anything regarding COBRA to the exiting employee. It looks like I have to provide: California's "For Your Benefit" pamphlet (if terminated), HIPP Notice, and a Notice to Employee as to Change in Relationship. Do I not also need to provide a Certificate of Group Health Plan Coverage?

| Find out from your insurance carrier, but typically, once you notify your insurance carrier that an employee has been terminated, they automatically send the employee the Certificate of Group Health Plan Coverage. You also need to provide them with the following COBRA documents (which can be found on this website): General Notice of COBRA, COBRA Continuation Coverage Election Notice, Acknowledgement of Receipt of Notification of COBRA Rights. I also recommend including a Final Paycheck Worksheet and a Final Paycheck Acknowledgement.
