| Is it legal to require all employees in a specific category (i.e. all "full-time, regular employees") to participate in an insurance benefit (like LTD coverage) when the premium is paid via payroll deduction from the employee?

Outside of collective bargaining agreements, you cannot force employees to pay for an insurance in which they do not want to or dont participate.
